Who We Are
Smile for a Lifetime Foundation is a charitable non-profit organization that provides orthodontic care to individuals who may not otherwise have the opportunity to acquire assistance. Launched in 2008, Smile for a Lifetime Foundation aims to reach individuals with financial challenges, special situations, and orthodontic needs. The Foundation sponsors the orthodontic care of hundreds of patients each year. Smile for a Lifetime Foundation has participating orthodontists throughout the US. Each chapter has its own local Board of Directors who chooses patients to be treated by the Foundation.
Who Qualifies
To become a candidate, interested patients must complete or have completed for them, a Smile for a Lifetime Foundation application that is available at the bottom of this page.
Our Mission
At Smile for a Lifetime Foundation it is our mission to create self confidence, inspire hope, and change the lives of children in our community in a dramatic way. The gift of a smile can do all this for a deserving, underserved individual who, in turn, can use this gift to better themselves and our community.
The Doctor
Dr. John Pobanz grew up in the Ogden area and completed his pre-dental training at Weber State University and Utah State University. He completed his dental education, with highest distinction, at the University of Nebraska Medical Center, College of Dentistry in Lincoln, Nebraska. He earned his two-year orthodontic specialty training, as well as a Master's degree in Oral Biology at the same institution. Upon graduation in 1998, Dr. Pobanz joined the practice of his father-in-law, Dr. Alan Christiansen, and assumed ownership in 2000. Dr. Pobanz is a Diplomate of the American Board of Orthodontics.
Our mission statement is "We are committed to becoming BETTER every day." Our patients are our priority and are treated with care and respect from their first phone call to their last appointment from a compassionate team of trained professionals.
Our team is involved in all aspects of orthodontic technology. Dr. Pobanz enjoys speaking to orthodontic teams about practice enhancement, team building and clinical excellence. Integrating innovative TAD mechanics, Suresmile, and the Damon philosophy are his specific clinical interests. Dr. Pobanz enjoys training for triathlons and traveling with his wife Cori. He also enjoys serving in the Boy Scouts and coaching his three kids Joel, Jane and Beth in sports.
Application & Questions
- Letters of Recommendation are mandatory. Please do not submit more than two letters, and limit each reference letter to one page each. Please type or print clearly with black ink (no pencil).
- Your attached picture of the applicant's teeth must be clear.
- Your application, letters of reference and pictures will not be returned to you and will become property of Smile for a Lifetime foundation.
- The applicant must be a resident of the area the foundation serves.
